Safety Reporting & Concussion Management Program

Program Summary

The Lakeland Hawks Hockey is committed to providing a safe and healthy environment for every player. In accordance with Wisconsin State Law (Act 172) and the policies of USA Hockey and the Wisconsin Amateur Hockey Association (WAHA), our organization follows a strict Concussion Management and Injury Reporting Program.

This program ensures that every player, coach, and parent understands how to recognize, report, and respond to potential head injuries or other serious injuries sustained during practices or games.

Our goal is simple: when in doubt, sit them out and report it immediately.

Recognizing and Reporting a Concussion

Concussions are serious injuries that can occur from any bump, blow, or jolt to the head or body that causes the brain to move within the skull. Even a seemingly minor hit can result in significant symptoms.

A concussion may affect a player in the following ways:

  • Physical: Headache, dizziness, nausea, fatigue, light or noise sensitivity.
  • Cognitive: Confusion, difficulty concentrating, slow response time, memory issues.
  • Emotional: Irritability, mood swings, unusual behavior.
  • Sleep: Trouble falling asleep or sleeping more or less than usual.


Over 90% of concussions do not involve loss of consciousness. Recognizing subtle signs is key.

Concussion Action Plan

If a concussion is suspected, either by a coach, parent, or player:

  1. Remove the player from play immediately.
  2. Do not allow the player to return to practice or competition that day.
  3. Report the incident by completing the Lakeland Hawks Concussion Report Form.
  4. Submit the report to the Lakeland Hawks Safety Officer at durk_a80@yahoo.com.
  5. Seek medical evaluation from a licensed healthcare provider experienced in concussion management.
  6. Provide medical clearance to the Safety Officer before the player returns to the ice by submitting the completed Player Return Form.

No player may resume play until written clearance from a qualified healthcare professional is received and reviewed by the Lakeland Hawks Safety Officer.

Parental Responsibilities

During registration, all parents and guardians must acknowledge they understand Wisconsin Act 172 and USA Hockey’s Concussion Management Program. This includes agreeing to follow all safety protocols and respecting the return-to-play process following any suspected or confirmed concussion.

Injury Reporting and Return-to-Play (Non-Concussion Injuries)

In addition to concussion procedures, the Lakeland Hawks require documentation for any injury that requires medical attention and prevents a player from practicing or competing.

Steps for Reporting and Returning:

  1. The parent or guardian must complete the Lakeland Hawks Injury Report Form and submit it to the Safety Officer.
  2. The injured player must be cleared for return by a licensed physician. The written medical release must include the return date and any activity limitations.
  3. Email the completed Player Return Form to durk_a80@yahoo.com.
  4. The Safety Officer will notify the coaching staff and the Lakeland Hawks Board once the player is cleared to resume participation.

The Lakeland Hawks Hockey  takes player safety seriously. Every coach, parent, and athlete shares responsibility for recognizing and reporting potential injuries. Early reporting and proper care not only protect the player’s health but also promote a stronger, safer hockey community.
